Uniform Patrol Division: How to File Criminal Charges

The filing of criminal charges can be an unsettling task. We hope that you will find this information helpful.

File charges against is an ADULT (18 years old or older):

  • Go to the Municipal Court Prosecutor's Office and pick up a prosecution packet. The office is located at 50 East Columbia Street on the 4th floor. (Packets can be picked up between 9:30am and 11:30am and between 2:00pm and 4:00pm Monday through Friday.)
  • Fill out the required information in the prosecution packet.
  • Return the completed packet to the Municipal Court Prosecutor's Office for review.
  • Following review by a prosecutor, you will be notified of what action will be taken, if any.
  • If a summons is issued, you will be notified by mail as to when you are scheduled to appear in court. If a warrant is issued, once the warrant is served you will be notified as to when you are scheduled to appear in court.
  • Information that you will need to file charges:
    1. Copy of the police report. (report is free of charge and may be obtained at Springfield Police Headquarters located at 130 North Fountain Avenue between the hours of 8:30am and 4:30pm Monday through Friday)
    2. Date and time of the incident.
    3. Suspect name and address.
    4. Witness names, addresses and statements.
    5. Medical reports, if applicable.
    6. Photographs of injuries/damages.
    7. Estimate of damage/loss.
    8. Copy of court order, if applicable.

File charges against is a JUVENILE (under 18 years old):

Unruly activity is defined as behavior that, if committed, would not be a crime if committed by an adult (truancy, disobedience toward parents, etc.)

  • Prior to filing the charge of Unruly against any juvenile (except runaways), the family must seek an assessment at Youth and Family Connections, 1835 Miracle Mile Road, Springfield, Ohio 45505, phone ( 937) 390-7964.
  • The charge of Unruly may be filed against a runaway prior to seeking an assessment.
  • If an assessment has been sought in the past, file the charge of Unruly by using the procedure for filing Delinquent charges below.

Delinquent activity includes any violation of the Ohio Revised Code (ORC).

  • Go to the Clark County Prosecutor's Office and pick up a prosecution packet. The office is located at 50 East Columbia Street on the 4th floor.
  • Fill out the required information in the prosecution packet.
  • Return the completed packet to the Clark County Prosecuto's Office.
  • Information that you will need to file charges:
    1. Copy of the police report, if applicable. (report is free of charge and may be obtained at Springfield Police Headquarters located at 130 North Fountain Avenue between the hours of 8:30am and 4:30pm Monday through Friday)
    2. Date and time of the incident.
    3. Suspect name and address.
    4. Witness names, addresses and statements.
